CommunityConnect HealthPlan (CCH) Website Statement Regarding Collection and Use of Information
Welcome to CommunityConnect HealthPlan’s website. We appreciate your interest in us. This site is owned and operated by CCH and its affiliates. Any activity on this site is subject to monitoring by CCH at any time. CCH routinely gathers quantitative information on website activity, including the number of users to the site and the pages visited. No personally identifiable information is collected in this routine activity, and subsequently, CCH reserves the right to use and share or disclose this information with others.
When you log on to our website and communicate with us via our site, we do not collect personal information about you unless you provide it to us voluntarily. Personal information is any information or data that is unique to an individual such as a name, social security number, address, birth date, etc. Portions of our website may allow you to provide us with personal information in order to deliver requested materials, products or services to you, respond to your questions, or enable to you to update information or sign up for CCH programs. Any personal information that you provide via our website is used exclusively for the purpose stated and contained on the website page. CCH does not sell, license, transmit or disclose personal information that you provide to us outside of CCH and its affiliated companies except with the following exceptions:

HIPAA Notice of Privacy Practices
The Health Insurance Portability and Accountability Act of 1996 (HIPAA) Privacy Rule allows members the right to receive a notice that describes how individual health information may be used and/or disclosed and how to acquire access to this information. CCH and its affiliated health plans are federally mandated to send a notice (Notice of Privacy Practices) to members of its fully insured health benefits plans.

CCH and its affiliated health plans have developed the following Notices of Privacy Practices based upon federal and individual state regulations.
PRIVACY AUTHORIZATION FORM
CCH is fully committed to the spirit and letter of the Health Insurance Portability and Accountability Act of 1996 (HIPAA), including but not limited to the Privacy Rule that was issued pursuant to HIPAA. A major provision of the Privacy Rule is to safeguard sensitive, personal information about members. This information is referred to as Protected Health Information (PHI), and includes individually identifiable health care and demographic data.
HIPAA allows CCH to use and disclose certain member information for clearly defined treatment, payment and health care operations (TPO). However, members have the right to restrict the release of information about them beyond these uses. If you need to have your PHI disclosed to someone outside of CCH for purposes other than TPO or have questions, contact the customer service telephone number on your member identification (ID) card.

Providing personal information through CCH's website is an option. Additionally, personally identifiable information will not be collected from you without your knowledge and approval.
