Vision Services 

We cover:

  • Eye exams and eyeglasses
  • Treatment for diseases or injury to the eyes

Dental Services

BadgerCare Plus Standard Plan

  • Full coverage with no copay

BadgerCare Plus Benchmark Plan

  • Coverage limit for preventive, diagnostic, simple restorative, periodontics and extractions (surgical services) for pregnant women and children
  • Coverage limit of $750 per enrollment year
  • A $200 deductible applies to all services except preventive and diagnostic
  • Cost-sharing equal to 50% of allowable fee on all services
  • Note: Pregnant women do not have to pay a deductible or share the cost for dental services

CommunityConnect covers dental services. But you must go to a CommunityConnect network dentist. See the provider directory or call our CommunityConnect Member Advocate Call Center to find a dentist near you at 1-888-279-1227; 1-800-947-3529 (TTY). As a member of CommunityConnect, you have a right to see a dentist for a routine visit within 90 days after you ask for it.

Dental Emergency

It is a dental emergency when you need to see a dentist right away to treat dental pain, swelling, fever, infection or injury to the teeth. If your dentist is with CommunityConnect:

  • Call your dentist's office.
  • Tell the staff that you or your child has a dental emergency.
  • Tell the staff what your dental problem you have, such as a toothache or swollen face. Make sure the staff knows that you or your child has a “dental emergency.”
  • Call us if you need a ride to your dentist’s office.

If your dentist is not with CommunityConnect:

  • Call CommunityConnect. Tell us that you or your child has a dental emergency. We can help you get dental services.
  • Tell us if you need a ride to the dentist‘s office. We can help get you a ride there.

You have the right to get treatment for your dental emergency within 24 hours after you ask us for it.

If you have a dental emergency and need help finding a dentist, you also can call Southeast Dental Associates at 1-877-389-9870. Members with hearing or speech loss may call the Southeast Dental Associates TTY line at 1-800-947-3529.

Some services need an OK from CommunityConnect and/or your Primary Care Physician (PCP) (prior authorization) for us to cover the service. You must go to a provider in your network and the service must be needed for medical reasons (medically necessary). You must get an OK for these dental services:

  • Dental oral surgery (such as for TMJ)
  • Inpatient facility
  • Anesthesia services
