How to Enroll
CommunityConnect HealthPlan is the new health plan choice for BadgerCare Plus members living in Kenosha, Milwaukee, Ozaukee, Racine, Washington and Waukesha counties. Not only does CommunityConnect cover services for medical care, mental health and dental, we have many programs to help you manage your health issues.
Enrollment
You have a few ways to sign up for BadgerCare Plus:
- Apply online at the BadgerCare Access website
- Call us: 1-800-362-3002
- Fill out the below form following the instructions in the form: dhs.wisconsin.gov/forms/F1/F10182.pdf
- Go to your local county or tribal agency for an interview
You also can get help with your application through Community Advocates or other community organizations. For a list of community organizations that can help in your county, click here.
To learn if you are eligible for BadgerCare Plus, visit BadgerCare Access website to find out if you might be eligible.
You must choose one HMO for all family members. If you do not, the Department of Health Services will choose a plan for you. You may change plans within three months and then you must stay with that plan for the next nine months.
These are choices for current members to choose an HMO. To enroll in BadgerCare Plus and confirm eligibility, apply online at access.wi.gov or call 1-800-291-2002.
