CommunityConnect HealthPlan teams up with Community Advocates to help improve the health of our members in and around Milwaukee. Community Advocates’ knowledge in the area helps CommunityConnect build strong connections among our members, health care providers and us.

Outreach: Take Into Account Member Needs
- Create trusted relationships. Our outreach workers connect members with community support programs that address basic needs and benefits, plus promote healthy lifestyles.
- Communicate clearly.
Health Education: Share About Better Care
- Help members know their health care benefits. We know it can sometimes feel like it is all too much. Community Advocates is here to talk one-on-one with members about benefits and special programs. These include well-care visits, routine tests and vaccines as well as programs to help manage your health concerns.
